Your submitted fields are sent for the purpose of responding to the enquiry. Do not use the form for emergencies or send confidential attachments.

Cloudflare Turnstile is used to distinguish legitimate submissions from automated abuse. Cloudflare processes technical information according to its own privacy terms.

Analytics status: this site does not currently have a Google Analytics measurement ID configured. If measurement tools are introduced, this statement should be updated before or when they are enabled.

You can ask questions about your information—not just trust that it is handled properly.

Subject to legal and professional limits, clients may ask to access their personal health information, request correction of information they believe is inaccurate or incomplete, withdraw consent where consent is the basis for handling information, and raise a privacy concern.

Identity may need to be verified before access or correction requests are processed. Some information may need to be retained despite a withdrawal request because of legal or professional record-keeping obligations.

Policies, informed consent, limited access, service-provider review, and procedures for responding to privacy concerns or breaches.

Technical

Appropriate account controls, secure platforms, device protections, backups, and security monitoring according to the service and risk.

Physical

Reasonable protection of devices, workspaces, and any paper records against unauthorized access, loss, or disclosure.

No method of electronic transmission or storage can be guaranteed completely secure. If a privacy incident occurs, the practice will assess it and take steps required by applicable law and professional obligations.
